
For those interested you need to apply for the birth certificate at the office of the Chief Registrar of Birth & Death that's associated with the ward in which hospital that the birth occurred in, is part of . Some hospitals have a Government desk attached to them but in case there isn't you can ask the hospital to point you in the right direction.You can also check out the List of Officers and Registration Centers at the BMP website
Documents you need - A certificate issued at the hospital attesting the birth and the Birth Certificate application form. If you want to include the name of the baby in the Birth Certificate then there is another form to fill. It is recommended that you apply for this certificate within 21 days of the birth
